The terms and Conditions:
1. Introduction :
. Salemcts is a Non-Emergency Medical Transportation service
. Based in Winston-Salem North Carolina since 2015
. Effective date of terms and conditions 1/01/2024.
2. Definitions:
. Salemcts Transportation provider of NEMT services
. Trip destination one way = one trip
. Round trip destination and back to pick to pick up point = two trips
. Client Person being picked up for transportation.
. Booking setting up NEMT services
3. Booking and Reservations:
. Booking a NEMT service can be done at this website, email salemctsride@gmail.com
. Booking can be done by phone at 336-870-7635
. Cancellation policy: must be given over the phone within 24 hours before scheduled pickup.
4. Transportaion NEMT Services:
. Ambulatory clients who can walk without assistance
. Wheelchair clients who need the assistance of a wheelchair.
. Hours of service a available daily from 8am to 5pm regular hours Monday - Friday
. Premium availability any time before 8am or after 5pm also weekends and Holidays
. Any destination outside of the 10-mile radius, will incur a premium mileage charge to and from.
5. Payment and Billing:
. Services will be based on a 10-mile radius from pick point to destination, same price returning.
. Payment methods accepted Master card, Visa, Cash app, or cash.
. late Payment or Non-payment consequences based on the client.
6. Expectation of clients:
. Be ready there will be a wait time charge if the wait is more than 15 minutes.
. Be civil and respectful. the driver can refuse the trip, due to improper behavior.
. Clients must provide accurate information and contact details.
7. Transportation Provider Responsibilities:
. Timely and reliable service.
. All vehicles are maintained and clean.
. All drivers undergo a mandatory criminal background check/drug test.
. Drivers must pass a DMV background check.
8. Privacy and Confidentiality:
. All patient private personal information (PPI) will protected following HIPPA guidelines.
. This information includes all identifying details such as an address, destinations, medical condition, etc...
9. Insurance:
. Salem cts is fully insured to meet NC state limits for business and vehicle insurance.
. Including general liability coverage for accidents or incidents.
10. Termination of Services:
. Termination of services at the request of the Clients
. Failure to pay for services provided.
. Failure to qualify for services, such as the total allowed trip for the Client's insurance is exhausted.
11. Amendements and Updates:
. When changes to the terms and conditions will be communicated through emails, direct communications with providers, and through our website.
. All changes will go into effect 30 days from the initial notification.
12. Acceptance of Terms:
. By booking with Salem cts Services Client agrees to abide by these terms and conditions.
Return and Refund Policy to clients:
1. Booking and cancellation policy:
. Clients are responsible for providing accurate and complete booking information, including date time and destination.
. Cancellations made within 24 hours of the scheduled pick-up time will be eligible for a full refund.
. Cancellation made after the specified time frame may be subject to a cancellation fee, which will be deducted form the refund amount.
2. Refund Eligibility:
. Refunds will be considered for the following situations:
. a. Cancellation made within the 24-hour time frame.
. b. Fallure to provide transportation services due to reasons beyond our control (e.g., vehicle breakdown)
. c. Incorrect charges or billing errors.
3. Requesting a Refund:
. Clients must submit a refund request in writing to {refund request email or phone}
. Refund requests should include:
a. Booking reference number.
b. Reason for the refund request.
c. Supporting documentation, if applicable.
4. Refund Process:
. Upon receiving a refund request, our team will review it within 7 business days.
. If the refund is approved, the refund amount will be processed using the original payment method whenever possible.
. Refunds may take 5-10 business days to reflect in the client's accounts.
5. Non-Refundable Fees:
. Certain fees may be non-refundable, including but not limited to booking fees, administrative fees or fees associated with third-party services.
6. Dispute Resolution:
. In case of a dispute regarding a refund request, clients may contact our customer support team an email at salemctsride@gmail.com.
